We regret this was un-understandable.  Maybe the following sheds some light, maybe not:

The patient gets the bad news about his lumbar disc herniation, a serious lower back injury that requires surgical intervention.  Then the MD catches the patient eating pizza, which isn't allowed before surgery, but the patient tells the MD to just take it out while the MD is "in there" (during the impending surgery).  Then the patient tells the MD to just take everything out, right through the patient's nose, the "full mummy", and the MD finally agrees to do it if the patient signs a release. 

Later, in the surgery, the MD sucks everything out through the patient's nose with a Dyson vacuum cleaner, using the hose attachment. 

In recovery, the patient wakes up, feels a lot safer with all his organs out where he can see them, and takes them home in a carryout box.  The nurse reminds him to eat plenty of fish for omega 3 fatty acids.  He complies.
